Online Health Assessment


Welcome to the Everyone Health Stop Smoking online health assessment.


The assessment will take approximately 10 minutes to complete and it will help us understand your health needs and how we can best support you to achieve your healthy goals.


By the end of the assessment we will be able to understand and prioritise your needs.


We can then either refer you directly into the Everyone Health service or signpost you to other relevant services or resources that will support you.


During the assessment you will be asked to create an account with dedicated username and login which will allow you to easily access your assessment and any plans or services we may agree on.
